These Terms and Conditions (“Terms”) govern your access to and use of the Fuel by Design website (“Site”) and any related online services, scheduling, forms, messaging, and telehealth offerings (collectively, “Services”). The Site and Services are operated by Fuel by Design (“Fuel by Design,” “we,” “us,” or “our”).
By accessing or using the Site or Services, you agree to these Terms. If you do not agree, do not use the Site or Services.

We provide wellness and medical services by appointment, including in-person visits in Warren, Ohio and limited telehealth services where permitted by law.
2) Medical Disclaimer (Important)
- Educational content only: Content on this Site is provided for general informational and educational purposes and is not medical advice.
- No diagnosis or guarantees: We do not guarantee outcomes. Individual results vary based on many factors.
- Not for emergencies: Do not use this Site or Services for emergency needs. If you believe you have a medical emergency, call 911 or go to the nearest emergency room immediately.
3) Eligibility
You must be at least 18 years old to use the Site and Services unless a parent/guardian provides consent for any permitted communications or scheduling (where applicable). You agree that all information you provide is accurate and current.
4) Appointments, Scheduling, and Communications
Appointments may be requested through our Site, forms, or scheduling links. Appointment availability is not guaranteed until confirmed by our team.
You agree that we may contact you at the phone number or email you provide for scheduling, care coordination, reminders, service updates, and administrative messages. Standard messaging/data rates may apply.
5) Telehealth Terms & Consent
If you request or participate in telehealth, you understand and agree:
- Consent: Your consent to receive telehealth may be obtained and documented prior to services and billing, and you may be asked to acknowledge telehealth-specific informed consent during intake or at the start of your visit.
- Location matters: You must be physically located in a state where the clinician is authorized to provide telehealth services at the time of the appointment. If you are not in an eligible location, we may reschedule or convert your visit to an in-person appointment (if available).
- Technology limits: Telehealth depends on internet/technology and may be interrupted. If a telehealth visit cannot be completed, we may attempt to reconnect, reschedule, or recommend appropriate in-person care.
- Appropriate care: We may determine telehealth is not appropriate for your needs and recommend in-person evaluation or referral.
6) Payments, Fees, and Purchases
Fuel by Design is primarily self-pay unless otherwise stated. Fees may include consults, follow-ups, memberships (if offered), labs, and other clinical services.
Pricing anchor: The initial clinical consultation is typically $299. Labs, medications, and ongoing care vary by plan and clinical needs.
By purchasing Services, you authorize us (and our payment processors) to charge your selected payment method. If a payment fails, we may pause services until payment is resolved.
7) Cancellation, Reschedule, and Refund Policy
Because we reserve clinical time specifically for you, the following policy applies:
- 24-hour notice: Please provide at least 24 hours’ notice to cancel or reschedule.
- Late cancellations: Cancellations or reschedules with less than 24 hours’ notice may incur a late cancellation fee.
- No-shows: Missed appointments may be charged in full.
- Non-refundable once rendered: Clinical consultations, administrative services, and medical services are non-refundable once delivered.
- Non-refundable once ordered/dispensed: Labs, medications, supplements, compounded products, and shipping are non-refundable once ordered, dispensed, or shipped.
- Prepaid, not-yet-rendered: Prepaid services not yet delivered may be reviewed case-by-case at our discretion, subject to administrative costs and third-party fees.
8) Prescription Eligibility & Clinical Decision-Making
If medication is discussed, you understand:
- Medication is not guaranteed and depends on clinical appropriateness, your health history, screening, and labs (as applicable).
- We may recommend lifestyle, nutrition, training, sleep, and recovery strategies with or without medication.
- All treatment decisions are made at the clinician’s sole professional discretion and in accordance with applicable standards of care.
9) Privacy and Your Information
Our handling of personal information is described in our Privacy Policy and, where applicable, our Notice of Privacy Practices. If you become a patient, health information may be handled under applicable privacy laws and policies.
10) Testimonials, Reviews, and Typical Results
Testimonials and reviews reflect individual experiences and are not a promise of what you will achieve. Any “before and after” statements are not typical unless clearly stated and supported. We do not guarantee results.
11) Acceptable Use of the Site
You agree not to:
- Use the Site for unlawful purposes or in violation of any applicable law or regulation.
- Attempt to access, probe, or disrupt Site security, hosting, or related systems.
- Upload malware, scrape content, or interfere with Site performance.
- Misrepresent your identity, health information, or eligibility for services.
12) Intellectual Property
All content on the Site (text, graphics, logos, images, videos, layout, and design) is owned by Fuel by Design or used with permission and is protected by intellectual property laws. You may view, download, and print pages for personal, non-commercial use only.
13) Third-Party Services and Links
The Site may link to third-party websites or tools (e.g., scheduling, payment processing, forms). We do not control third-party services and are not responsible for their content, policies, or practices. Your use of third-party services may be governed by their terms and privacy policies.
14) No Warranties
The Site and Services are provided on an “as is” and “as available” basis. To the maximum extent permitted by law, we disclaim warranties of any kind, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement.
15) Limitation of Liability
To the maximum extent permitted by law, Fuel by Design will not be liable for indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of the Site or Services.
Our total liability for any claim related to the Site or Services will not exceed the amount you paid to Fuel by Design for the specific Service giving rise to the claim, if any, during the three (3) months preceding the event at issue.
16) Indemnification
You agree to defend, indemnify, and hold harmless Fuel by Design from claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising from your misuse of the Site, violation of these Terms, or violation of any law or third-party rights.
17) Changes to These Terms
We may update these Terms from time to time. Changes are effective when posted to this page with a new “Last Updated” date. Your continued use of the Site or Services after changes are posted means you accept the updated Terms.
18) Governing Law
These Terms are governed by the laws of the State of Ohio, without regard to conflict of law principles.
19) Severability
If any provision of these Terms is found unenforceable, the remaining provisions will remain in full force and effect.
